It's appropriate for a man to extend his hand and shake the woman's hand. This also goes for Canada.
.....IN the SouthEastern parts of the US it is considered rude and brash to offer your hand to a woman..unless she offers her first. Its as if you are putting her on a pedastel and she is allowing you to shake her hand.
